The following paperwork is required
- Health Assessment form
- Completed Immunization record
- Original Birth Certificate (we will copy and return)
- Proof of residency (can be a driver’s license, bill, mail)
If you are unable to bring something on registration day, please bring these items as soon as you can- we are open Monday-Thursday in the summer months!
